If it doesn't lend itself you have to reach in and grab it. I usually do it with the T; ROTFLAMO, rot-fl-mao. Pronounces real easy where I come from.

And, I have only heard LOLcats pronounced /'lɑlkęts/, never /ʔɛlʔoʊʔɛl#kęts/.
