Looks like a style manual question. I'd say they're all correct but if you're submitting to a publication, the style manual of the publication may state a preference for one over the other. Faldage, you're dead right. Publishers have their own preferences. E.g., internal thoughts of a character: in italics, or in quotation marks. Nit-picky, but important to the publisher.
