Hi,
I found this a most incredible story. From a scientific standpoint, the way to avoid such a blunder would be to stop asking the same questions to all 1.8 million candidates. The consequences of the intrinsic fuzziness of language could thus be statistically attenuated. Either the number of re-scored tests would be reduced, or nobody would insist on it, since it would concern fewer people.
