It's similar to a smock isn't, except perhaps that a smock keeps muck from touching that underneath it, and a johnny seems to keep it from escaping :-\

I read an article a little while back about a firm that has started to make more, well, complete johnny versions (ones with backs to them!). The motivation for the new design was at first to accomodate modest women in Muslim countries, but the rework ended up finding many fans all over the world.
Apparently people aren't keen on the idea of walking around in hospitals with a bare derrière.
