When ever I have heard or used the term "having a bad hair day" it has always meant just the literal meaning. What do others think?

My thought exactly, Steve. I don't think I've ever heard it used in a broader context. Welcome aBoard!

Since guys, generally, don't care about the appearance of their hair it is always in a broad context.

guys always care about the appearance of their hair.
they care more, even about the disappearance!

(they might not care about hat hair, but that is another story)

Head hair is almost always desirable.
when it's insufficient, many a man shaves what remains off, electing to have none rather than some.

Body hair is an other story.
Young males, in movies, have arm pits as devoid of hair as a starlet--for the most part, hairlessness is now a requirement for leading men..
Unless they are baddies.. bad guys can often be identified by their body hair, be it facial (stubble, or stashes) or their lack of it (shaved heads can be neutral or size of danger.)

and just last month, there were late night comics making comments on brittany spears and cohorts and their lack of hair down there...

I've occasionally heard it used because "I'm in a bad mood because all the little things are going wrong and if something doesn't go right soon you really don't want to be around me" takes too long to say.
But usually it is just about hair.
